General Requirements for New Customer/Payroll/Maybank Vendor Partners:

1. Company asset ≥ IDR100 billion
2. Number of employee  ≥ 50
3. Company is not listed on black list external or intrenal
4. Listed in Bursa Efek Indonesia
5. Length of business  ≥ 3 years
6. Profit in the last 3 years  (for new Customer)

Required Documents:

1. Credit card application form (original)
2. Copy of SIUP & TDP
3. Copy of NPWP
4.  Audited Financial report of last 3 years
5. Recommendation letter from business unit (for payroll company / vendor partner)
6. List of corporate cardholder:
   - Name
   - Title/level
   - Credit limit
7. Copy of KTP (Indonesian)/passport & KITAS (foreigner) :
   - Board of Director
   - Board of Commisioner
   - Corporate Cardholder

General Requirements for Funding Customer:

1. Has recommendation from business unit
2. Corporate Card facility  is stated on Agreement
3. Company asset ≥ IDR100 billion
4. Number of employee  ≥ 50
5. Not listed on the internal or external blacklist
6. Minimum length of business is ≥ 3 years
7. Has a good credit history at SLIK

Required Documents:

1. Credit card application form (original)
2. Copy of NPWP
3. Copy of SIUP & TDP
4. Copy of  article of incorporation and changes (if any)
5. Audited financial report in last year
6. Recommendation from business unit
7. List of Corporate Card recipient :
   - Name
   - Title/level
   - Credit limit
8. Copy of KTP (Indonesian) /KITAS (foreigner)
9. For secured Corporate Card, additional document are :
   - Letter of authority for  account withdrawal
   - Letter of agreement penyerahan hak atas tagihan

General Requirements for Professional:

1. Has recommendation from business unit
2. Corporate Card facility  is stated on Agreement
3. Company asset ≥ IDR 100 billion
4. Number of employee  ≥ 50
5. Not listed on the internal or external blacklist
6. Plafond of lending is ≥ IDR 100 million
   - For new lending : Must profit in the last 3 years
   - For existing lending : current in the last 3 months

2. Copy of latest article of association
3. Copy of  Credit facility sheet
4. Recommendation from business unit
5. List of Corporate Card recipient :
   - Name
   - Title/level
   - Credit limit
6. Copy of KTP (Indonesian) /KITAS (foreigner)
